We are accepting applications for Preschool and for Grades Kindergarten to 7th Grade for the School Year 2024-25. The application process is open to all regardless of a family’s ability to pay tuition. Seton School has several tuition assistance options for families. Applications are available in the school office as well as on this website. First priority is given to returning students and siblings. Each student applicant is tested individually to properly assess grade placement. A testing fee is charged for each student applicant. All new parents who are applying for preschool as well as parents of children applying to grades Grades K-6, will be interviewed by the school principal prior to acceptance to Seton School.

THE BASIC FUND

Tuition Assistance for NEW students entering Preschool – 7th Grade is available by downloading the The Basic Fund Application and submitting it to the School with financial information no later than February 15, 2024.

For families that are already receiving assistance from The Basic Fund, your renewal will be sent to you in March 2024.

 

 

TADS

Everyone applying or enrolled at St. Elizabeth Seton must apply to TADS.  When you apply to TADS you are applying for the tuition assistance from the Diocese of San Jose and for the Archdiocese of San Francisco .  You must apply by February 10, 2024.   There is a $42.00 fee payable to TADS. If you are a returning family, please use your login information from this year. New families need to set up an account.

Applications to the above funds will determine your eligibility to apply to the St. Elizabeth Seton School Tuition Assistance Fund (SESS). If you would like to be considered for financial assistance from this program, you must be current with your school tuition payments and complete the application for this program by March 1st 2024.

Kindergarten Health Requirements

Kindergarten Immunization

Requirements This document will inform you about specific immunizations that are due when entering kindergarten. As of June 20, 2015 all students will be required to show proof of vaccination unless there is a medical exemption. Typically, incoming kindergarteners will need:

Their 5th DTap or TDap
Their 4th Polio
Their 2nd MMR
Their 2nd Varicella (chicken pox)

Report of Health Examination for School Entry

This form is required for kindergarten students and needs to be completed no more than 6 months before the start of school. The form is often referred to as: CHDP or Health Exam.

Oral Health Examination

This form is required by the State of California for kindergarten students and needs to be completed no later than May 31 of the kindergarten school year. This needs a Dentists signature and parent signature

TB Assessment

All incoming kindergarteners and any student transferring from outside of Santa Clara County will need to complete a TB Assessment Form with your doctor. This is a questionnaire to determine if your child needs to have a TB skin test. If no skin test is required, please return the signed form with the physicians’ signature. If a skin test is required, please return signed assessment form and the date the TB skin test was given and results by August 10, 2015. You can find the form at this link:

Medication Administration at School

This form will need to be signed by the physician and by the parent. This form lists the medication(s) that your student will need to take while at school.

It includes: time, dosage or amount of that medicine to be given, who may administer medicine to your child, and how much medicine to provide.

Please complete all lines on this form before bringing to the office.

**Please note: Medications are not allowed at school without this form. This includes Over-the-Counter medications such as, Tylenol and Advil.
